Melody Bar & Grill Rewards & Melody Approach: Rules

1. Sponsor
Melody Bar & Grill, 9132 S Sepulveda Blvd, Los Angeles, CA 90045 ("Sponsor"). [Sponsor to confirm legal business name.] App and game provided by SousShift AI.
2. One simple points program
- Joining Melody Bar & Grill Rewards is free. One account per person. Melody Bar & Grill has its own points balance; points don't move between restaurants.
- 10 points per $1 on eligible purchases (before tip). Until Toast is connected: tap "Add my visit" in the app and snap your receipt or type the check number, total and date (today or yesterday). One receipt per member per day, each check once, up to $150 per check. Claims are checked against the restaurant's sales report and points from made-up checks are removed. Once Toast is connected this is automatic and your points carry over. [Sponsor to list what is not eligible, e.g. tax, tip, gift cards, alcohol if required.]
- Bonuses, each capped: +100 once when you join · +250 once a year on your birthday · +100 per friend who joins with your code and makes a first visit · up to +50 a day from the game (see 3).
- There is no daily spin, no prize wheel, no lucky draw, no tokens and no second currency.
3. Melody Approach
Melody Approach is an arcade flying game with two modes. Campaign: 10 levels of about 45 seconds over LA, each a little harder, with six boss fights (levels 3, 5, 6, 7, 8 and 10), then a landing at LAX. Survival: no finish line; the game keeps getting harder until your plane goes down, and your time survived is what counts. You shoot down jets, UFOs, choppers and the bosses, dodge their fire (every attack is warned first), and catch Melody's food and music notes. Guns go live about 20 seconds into a flight; beating levels and bosses (or, in Survival, flying longer) unlocks stronger guns, which use limited ammo refilled by catching ammo crates. Three hits and your plane goes down (the game is over, nothing else happens). Players can tap the ⚔️ button to switch to a calm flight with no combat.
- Free to play, with or without an account. No purchase necessary.
- BETA: game rewards are switched OFF until Sponsor turns them on in its back office. While they are off, the game gives no prizes, codes, discounts or rewards points; the weekly boards rank players for bragging rights only.
- Two weekly boards, both reset every Monday: Campaign (ranked by score) and Survival (ranked by the longest time survived). Practice and test flights don't go on the boards.
- Once Sponsor turns rewards on: your score turns into rewards points (1 point per 100 score, at most 50 game points a day, Los Angeles date), and the top player on each weekly board may win the weekly prize shown in the game (for example, free Melody Fries). [Sponsor to confirm the prize, how winners are contacted and how it is redeemed.]
- Scores made with cheats, scripts, bots or multiple accounts don't count.
Melody Games Pack
- Trivia, Sports Trivia, Melody Word (one daily 5-letter word, the same for everyone that day) and Melody Mix (word scramble). Free, no purchase necessary.
- Once Sponsor turns game rewards on: Trivia and Sports Trivia give 1 point per 2 right answers (up to 5 a round), Melody Mix 1 point per 2 words (up to 3), and solving the daily Melody Word 3 points (2 points if you used 1 hint, 1 point with 2 hints; up to 2 hints per word). At most 10 Games Pack points a day per member and phone (Los Angeles date), and they count toward the 50-a-day game limit above.
- Truth or Dare is just for fun: no points. Dares never involve drinking, and anyone can pass.
- The LA Scoreboard shows live scores from ESPN for information only (no streams). "Who wins?" picks are free and just for fun: no money, no wagers. Once Sponsor turns game rewards on, a right pick made before the game starts earns 2 points, at most 6 a day, inside the 50-a-day game limit. Picks lock when the game starts.
Football Squares
No purchase necessary · 1 free square per member per game · extra squares with purchase.
- Free entry: every Melody Bar & Grill Rewards member (free to join, with a name) can get one free square on every board by tapping "Get my free square" in the app. No purchase is needed, and a purchase doesn't improve the odds of any one square.
- Extra squares with purchase: each eligible purchase can add one more square on the current board: show your Rewards card and staff add a Squares ticket (1 ticket = 1 square). Each board has a per-person limit set by Sponsor (for example 5 squares: 1 free + 4 with purchase), and a board never holds more than 100 squares or more players than its limit. There is no entry fee.
- One board per NFL or college game. Entries close at kickoff, when the row and column numbers (0-9) are drawn at random and shown. At the end of Q1, halftime, Q3 and the final (including overtime), the last digit of each team's score picks the winning square, using ESPN's scores.
- Prizes are set by Sponsor for each board and shown on it before you play: food, merch, % off the check, a free non-alcoholic drink, or % off a drink (21+ only, ID required). No cash, gift cards or money prizes. Never free alcohol (California ABC). If nobody holds a winning square, that prize rolls to the next quarter when the board says so; otherwise it isn't awarded.
- Prizes are claimed at Melody with the winner's Rewards card, follow the one-prize-per-day rule below, have no cash value and can't be transferred. Odds depend on how many squares are taken.
4. Rewards
Using a reward: tap "Use reward" and show the live pass to your server, who applies it in Toast. The points come off when you open the pass. A pass works once and expires 10 minutes after it opens; one reward per member per day.
One code per visit, one prize per day.
- One code per visit: every pass gets a fresh one-time code for that visit. It works once and expires 10 minutes after the pass opens. A used or expired code never works again; your next code comes on your next visit.
- One prize per day (Los Angeles date), per member and per phone: a reward, an early-bird / founding-member gift, or anything won in a game, spin or draw. Winning a gift and using that same gift the same day counts as one prize. A second prize that day shows "Come back tomorrow"; your next prize is available tomorrow. Games stay free to play for fun and score.
- Referral codes: one friend's code per member, entered once when you join; it can't be changed or swapped later, and you can't use your own. Referral points pay once per friend who joins with your code and makes a first visit. A referral never stacks with another reward on the same visit.
- These limits are checked by our server, not just your phone, so a second phone, a cleared browser or a screenshot doesn't get around them.

5. Changes
The Sponsor can change or end the program, the point values or the rewards with notice in the app. Points already earned stay in your balance unless the program ends. [Attorney: expiry and notice terms.]
6. Just Landed
One offer for everyone: 10% off your first order today when you show a boarding pass from a flight that landed at LAX in the last 24 hours. One per guest per day, can't be combined with other offers. [Sponsor sets the final offer.]
7. Nearby alerts and the one-time "Location buddy" bonus
- Optional. In the app, "Turn on nearby alerts" asks your phone for location and notification permission. Nothing is turned on without your tap and your phone's permission.
- What we collect: your phone's precise location (California treats it as sensitive personal information) and whether location and notifications are on. When: only while the app is open on your screen. A web app can't track you in the background, and we don't. Why: to show a "You're nearby" deal when you open the app within about half a mile of Melody Bar & Grill, and to count days for the bonus. Your exact location stays on your phone. The rewards system records, by date, that location and notifications were on, and an approximate location (about 1 km) each time you open the app (see 8). Never sold or shared, and not used for ads or any other purpose.
- Bonus: +100 points, once per member, when location and notifications were both on when you opened the app on at least 3 different days spanning at least 7 days. It's added automatically. It can't be earned again, including by turning location off and back on, using another phone, or rejoining.
- Notifications: on iPhone they need the app on your Home Screen (iOS 16.4 or newer). Any reminders we send are scheduled (for example lunch or happy hour); we can't send "you just walked past" alerts.
- Turning it off: tap "Turn off nearby alerts" in the app and/or switch off Location and Notifications for this site/app in your phone's Settings. Points you already earned stay. [Attorney: confirm CCPA/CPRA notice wording and the "limit the use of my sensitive personal information" approach.]
- The Sponsor can change the bonus amount or end it (see 5).
8. App opens (visit log)
- What's recorded each time you open the app (at most once every 10 minutes): the time, a short device summary (for example "iPhone · iOS 18 · Safari"), whether you opened it from your Home Screen, a random ID for this phone, and your member ID if you joined.
- Approximate location only if you've already turned on location for this app. The app never asks for location just for this. If location is on, it records one approximate spot (rounded to about 1 km) at the moment you open the app. Nothing runs in the background. Turn location off for this site/app in your phone's Settings to stop it.
- Why: Melody Bar & Grill's rewards (one prize per day, one account per phone), fraud prevention, and visit stats for the restaurant. Seen only by the restaurant's owner and SousShift AI, never sold and not used for ads.
